Note: Non-members can participate in this ride as a guest. If you are not a current Central Indiana Cycling member, you do not receive the excess medical ride insurance coverage provided to members. Please consider joining (and supporting) as a Member today! If you join as a member, you can skip this waiver form.

How often do I need to fill out this waiver? - You should fill this waiver out once per year. If your contact information or emergency contact has changed, please fill the waiver out again.
