Canines For Service

By signing this waiver, I hereby acknowledge that participation in the Canines for Service 5K / 1-Mile Event on October 11, 2026, at Long Leaf Park is a potentially hazardous activity that may result in injury or death. I understand that the event may involve running, walking, or other physical activity on public or private property, and that there are inherent risks, including but not limited to slips, trips, falls, collisions, dehydration, heat exhaustion, and other physical hazards.

I further acknowledge that Canines for Service works with trained service dogs, and I understand that animals may be present at or near the event. I accept any risks associated with interacting with or being in proximity to service dogs and other animals. 

Participants who bring a personal dog to the event do so at their own risk and are solely responsible for their dog's behavior, care, and supervision at all times. Canines for Service, its officers, directors, employees, volunteers, sponsors, and event partners are not responsible or liable for any injury, illness, loss, damage, or other incident involving participant-owned dogs, including injuries to or caused by such dogs. 

By signing below, I:

• Voluntarily agree to participate in the event.
• Assume all risks associated with my participation, including the risk of injury, illness, or death.

• Release and hold harmless Canines for Service, event organizers, sponsors, volunteers, staff, and other participants from any and all claims, damages, or liabilities arising from my participation, except for claims resulting from willful misconduct.

• Agree to abide by all event rules, safety instructions, and decisions of event officials, including the right to be denied or suspended from participation at any time.
• Certify that I am medically able to participate and have no known condition that would prevent me from doing so.
• Agree to respect all service animals present and not distract, touch, or interfere with them without permission.

I understand that this waiver is a legal agreement and that I have read and accepted its terms. I acknowledge that I am entering the event voluntarily and without coercion.

I also grant permission to Canines for Service and its representatives to use photographs, recordings, or other images of me taken during the event for promotional or educational purposes.
